Markham Plastic Surgery Website

I had created another website for one of the two Doctors in this practice, and he wanted to merge his site with the other Doctor's site, so I took the existing site you see and altered it to encorporate image and logos with my client.

I also created the contact form in the left sidebar to seperate out the two doctors so you can choose who to contact. Before it was one contact for the practice, and did not specify a doctor.
